Prior to your appointment, you will need to fill out consent and medical history forms. When your appointment is created, you will receive an email from our electronic patient chart system called Remedly

Please be sure to check your spam for an email from donotreply@remedly.com. You will have 5 days to use your temporary password, otherwise you will need to reset it by clicking on ‘Forgot Password.’
